We have done our best to simplify the Oklahoma State Permitting Process. Below is a form to submit all necessary information. Please fill out in it’s entirety. You will need to attach your Government ID, Birth Certificate, Proof of Experience, and BBP, First Aid, CPR certificate.
These responses will be used for us to complete your guest artist permit and Affidavit of Lawful Presence.
Once your information is submitted a confirmation message will appear with a link to pay the $50 Temporary Artist License Fee.

Artists already permitted in Oklahoma can forego everything below & email a copy of their current Artist Permit.

If you need a new BBP Certificate, we recommend this one, which is OSHA certified.
If you need a new FIRST AID Certificate, we recommend this one, which is OSHA certified.
If you need a new CPR Certificate, we recommend this one, which is OSHA certified.
